Magnum Portal

A web-based application to increase patient empowerment and seamlessly integrate patient health records

Patient Portal is an online application which aims to increase interoperability and data sharing between the primary healthcare stakeholders – the hospital, the doctor and the patient. It provides the hospital with an online platform for the maintenance of Personal Health Records for patients. Patients and doctors can use this unique tool to communicate and share vital data with ease and security over the Internet. Patients are offered a wide range of services through the portal, including personal health record review, appointment tracking, appointment booking and requesting, and invoice detail review.

 

Using Patient Portal, patients and care givers can:

  • Communicate with ease and security.
  • Take care of Pre-Registration, Pre-Appointment and Pre-Admission requirements.
  • Request new appointments and view upcoming appointments.
  • Regulate, view and restock medications.
  • Request, review and examine lab reports.
  • Examine current and past statements and other billing details.
  • Enter or modify personal information and other demographic information.
  • Review a list of prior referrals.
  • Maintain a comprehensive and up-to-date Personal Health Record. And much more.

 

Appointments:

This feature allows patients to book appointments with Care Providers, Services, and Clinics. Patients can also use this tool to re-schedule or cancel their appointments. In a bid to reduce human effort, a Virtual Visit feature has also been included, wherein a patient can consult with their physician via the Patient Portal itself.

 

Pre-Admission:

This section is intended for the performance of Pre-Admission functions. Details for a scheduled inpatient admission are recorded here, and patients can use this screen for confirmation regarding their eligibility for admission. This goes a long way in improving the hospital’s claims processing capabilities.

 

Prescription Services:

This feature allows patients to restock their medications. Users can set a Preferred List of Pharmacies, and have their medication delivered from these. Users can also perform self administration functions here, either by printing out the Medication Administration Chart or directly making entries in the patient portal’s online medication administration record.

 

My PHR:

This screen is the interface that a patient can use to view and keep track of any changes to their Patient Health Record. Patients can directly update their PHR with various details including allergies, eligibility details, and medications taken. The patient can also grant time-based access to his PHR to a care provider or a health care surrogate.

 

Investigation Results:

The Patient Portal also allows patients to view the results of Laboratory and Radiology Investigations. These results are uploaded by care providers after their approval from the concerned departments.

 

CCR:

Patient Portal also contains collaboration solutions to improve connected care and unification of data in pursuit of the broader goal of One Patient, One Record. If the care organisation has the facility to generate the patient’s Continuity of Care Record, the patient will be able to import details of all visits to disparate care organisations into his Personal Health Record on Patient Portal.

 

Statements Online:

Users can also use Patient Portal to view interim billing details of the current visit. The patient may also view a list of his past statements. The Patient portal provides patients with a highly secure transfer interface to make online payments of his Outstanding Bills.

 

My Inbox:

Patient Portal also helps the hospital maintain an active relationship with the patient through a dedicated messaging service. The hospital will be able to send reminders and patient relationship messages to the patient through Patient Portal informing the patient about the various services being offered by the hospital. This feature can also be used to provide the patient with patient care material and other details.

 

Patient Portal goes a long way in increasing a hospital’s Meaningful Use compliance, while reducing nursing labour burden, streamlining workflow and reducing human effort.
